I understand this program is strictly for participants aged 12 - 25, and I confirm the participant meets this age requirement.
I am the parent/guardian of the participant named in this registration form and I give permission for them to attend the Dungeons and Dragons activity (if the participant is under 18)
I understand this is a drop-in style program and supervision is provided only inside the venue during program hours.
I understand that participants may come and go from the venue as needed (e.g. to purchase food, take breaks).
I acknowledge the Shire and its volunteers cannot prevent participants from leaving the venue, nor escort them on or off the premises.
I understand that if the participant leaves the building, this is at their own discretion and risk.
I understand that staff/volunteers will not wait with the participant after the session ends, and punctual pick-up arrangements are my responsibility.
I understand that smoking, vaping, alcohol and any prohibited substances are not permitted at the venue or during program participation.
I understand that disruptive, unsafe or disrespectful behaviour may result in the participant being asked to leave.
I understand volunteers are not medical professionals and can only provide basic first aid until emergency services arrive.
I confirm the medical/emergency information provided is accurate and will be updated if it changes.
